Does the client have any medical or health condition? If YES, please provide full details (asthma, epilepsy, allergies, injuries, medical conditions, etc.): The Client/Parent confirms that the information provided is true and complete and agrees to inform the Academy immediately of any changes to health status.

Each client shall be assigned up to a maximum of two (2) coaches as determined by the Academy. Coach allocation, rotation, or changes are at the sole discretion of Pro Swim Academy, based on scheduling, availability, and program needs. The Academy does not guarantee permanent assignment to a specific coach.

Parents/guardians are not permitted to interfere with the coaching program, lesson delivery, or coaching instructions. Parents/guardians must wait only in the designated waiting area provided by the Academy. Staying next to the pool, giving instructions, coaching, or correcting swimmers is strictly prohibited. Any interference may result in: · Suspension of the lesson, or · Termination of services without refund.

All payments made to Pro Swim Academy are final and strictly non-refundable. Clients should not make payment unless they fully understand and agree to all Academy rules. A minimum of two (2) lessons per week is required. A standard package consists of ten (10) lessons, which must be completed within five (5) weeks from the first lesson date. Any lesson not used within this period shall be expired and forfeited with no refund or extension.

Clients wishing to continue must renew their package by Lesson 5 to retain: · Their coach (subject to availability), and · Their preferred time slot. Renewals are charged at the same rate. Due to high demand, the Academy strongly recommends booking a minimum of twenty (20) lessons to secure and retain preferred time slots. Failure to renew on time may result in loss of coach and/or time slot.

Clients must arrive on time for lessons. Late arrivals will only be taught for the remaining time. Cancellations must be made at least 24 hours in advance (or a minimum of 2 hours where expressly approved). Late cancellations or no-shows are counted as used lessons. Make-up lessons must be booked immediately after cancellation and are subject to availability. Make-up lessons are not guaranteed with the same coach.

Clients are strictly prohibited from: · Exchanging personal contacts with coaches, or · Communicating directly with coaches outside lessons. All communication must be conducted only through Academy administration. Clients must not engage coaches in private business, including sale of swimming kits, goggles, or equipment. Violation may result in termination of services without refund.

All clients must attend lessons with: · Proper swimming costume · Swimming goggles · Swimming cap
